In this role, you will perform comprehensive patient assessments, develop and implement evidence-based wound and ostomy care plans, and collaborate closely with physicians, therapists, and interdisciplinary teams. You will educate patients and families on wound prevention, ostomy management, and self-care, while documenting thoroughly in the EMR.
